Use a coaxial technique with the lumbar transforaminal technique. Similar to cervical transforaminal injection, proper use of fluoroscopy utilizing a coaxial technique improves the success rate of this block as well as patient comfort, as the needle is placed with minimal need of redirection. Once the needle is in final position, inject a small volume of radiographic contrast material using real-time, live fluoroscopic imaging to minimize needle penetration of an artery.
